6. For those without family or friends, replace “the person watching over you” with three simple things: a neighbor who can enter your home, a spot on the community outreach list, and emergency contacts saved on your phone.

Cost: No money is required. The hard part is mustering the courage to reach out — simply tell a neighbor or a member of the residents’ committee, “I live alone and have just experienced a crisis.”

In plain terms: When you have no relatives or friends, two things matter most: ensuring someone notices if something goes wrong, and making sure you eat and take medication on schedule. You can rely on yourself for meals and meds — just write it down and set reminders instead of trusting your memory. To guarantee someone notices, do three things: leave a spare key or temporary lock code with a neighbor, inform the residents’ committee that you live alone so they can add you to their outreach list, and save emergency contacts in your phone while keeping it powered on.

Benefit: A total of 44,573 outpatients with a history or risk of atherosclerotic thrombosis were enrolled in the international REACH study between December 2003 and December 2004; they were followed for four years. Of these, 8,594 participants (19%) lived alone. Over the four‑year period, the overall mortality rate for solo dwellers was 14.1%, compared with 11.1% for those living with others; cardiovascular deaths occurred in 8.6% versus 6.8% respectively. Both figures yielded log‑rank P‑values below 0.01, indicating the difference is unlikely to be coincidental. The magnitude of this effect varied by age: among people aged 45–65, solo living raised mortality from 5.7% to 7.7%; after adjusting for other factors, the hazard ratio was 1.24 (95% CI 1.01–1.51). For ages 66–80 the HR was 1.12 (1.01–1.26). In contrast, for those over 80 the rate fell to 24.6% versus 28.4%, giving an HR of 0.92 (0.79–1.06). In 2022, ten government agencies issued guidelines calling for regular outreach services for elderly individuals who are alone, living apart from family, disabled, or part of special family situations. These services — home visits, phone or video check‑ins, and remote monitoring — are to be organized by local townships and residents’ committees, who must also register each person’s willingness to receive help within existing community grids. By the end of 2025, at least one monthly visit must be guaranteed for all vulnerable seniors, and those who are completely dependent on care must receive even more frequent attention. The policy also encourages households to install smart call devices, water‑meter sensors, health monitors, and elder‑care alarms that can instantly alert emergency contacts when abnormal readings appear. Should any crisis be detected, staff are required to call emergency services right away.

Notes: Here is how to implement each of the three steps. First, give a spare key or temporary lock code to a neighbor or building manager and let them know you will be alone for a few days; no deep friendship is needed, just someone who knows you’re home. Second, approach the residents’ committee to request inclusion on the outreach roster; elderly people living alone are explicitly targeted by the program, and you can also ask whether any free smart alarms or water‑meter gadgets are available locally. Third, program your phone with emergency contacts and a medical ID card, and keep it on and unmuted during this period. The “B” rating stems from two factors. One, the link between living alone and higher mortality was identified through retrospective data analysis; it remains possible that poorer health or socioeconomic status predispose people to both solitude and earlier death, a confounding factor the original paper itself acknowledges. Two, the outreach policy applies only to seniors; younger adults lack comparable statutory support. However, the same document obliges community workers to “spot early signs of psychological distress such as family loss, unemployment, or school dropout” — a point echoed in Section 11, which notes that calling the 12356 helpline can provide needed companionship. Why having someone nearby matters is illustrated in Section 13, where cardiac arrest outside hospitals occurs in 79.2% of cases at home; survival rates jump from 3.9% when no one is present to 16.1% when a bystander performs CPR. Being alone therefore places you in the lower‑survival bracket. Additionally, arranging regular outings — such as daily trips to handle funeral paperwork, visit health clinics, or deal with civil affairs offices — can become part of your routine, making it easier to stay active. The helpline can be contacted repeatedly, not just once, as mentioned in Section 11. Finally, while living alone is a long‑term risk factor (Section 22 cites a hazard ratio of 1.32), for the immediate period described here the three practical measures provide the greatest protective benefit. 〔0.03〕 〔13.2〕 〔12.3〕
